Participating Insurances and their Requirements
Many insurance programs will cover the cost of surgery. However, it is your responsibility to confirm that your insurance company provides coverage for morbid obesity. Please keep in mind that some insurance carriers may have specific exclusion criteria or requirements to be met before weight loss surgery is covered.

Potential requirements for weight loss surgery include, but are not limited to:
- 6 Consecutive Months Structured Weight Loss Program
- 3 Consecutive Month Multidisciplinary Weight Loss Program
- Nutritional Evaluation
- Psychiatric Evaluation and Clearance
- Medical Clearance
- Cardiac Clearance
- Pulmonary Clearance / Sleep Study
- Lab results of TSH level & negative H-pylori
- 2 years of medical notes documenting BMI
- 5 years of medical notes documenting BMI

Request for insurance authorization for coverage of your surgery will be submitted after your evaluation to determine whether you are a surgical candidate. Once approved, the date of surgery will be determined between you and our surgical coordinator.
You may pay for the surgery yourself if insurance coverage is not provided. There are also several financing options available through participating banks.
